Consultation Agreement & Acknowledgment
Swift River Veterinary PLLC provides a consultative veterinary service designed to help pet owners better understand their pet’s health, nutrition, medical history, diagnostic information, and available conventional and integrative considerations. This service is intended to supplement—not replace—the care provided by your pet’s regular veterinarian.
By booking a consultation, I understand and agree that:
• My consultation is consultative and educational in nature and is not a substitute for an in-person veterinary examination or ongoing primary veterinary care.
• A telephone consultation with Swift River Veterinary PLLC does not establish a new veterinarian-client-patient relationship (VCPR) through the remote consultation itself.
• Swift River Veterinary PLLC will not prescribe or dispense medications, authorize refills, prescribe controlled substances, or independently direct me to discontinue or alter medications prescribed by my pet’s attending veterinarian.
• Recommendations may include discussion of nutrition, dietary therapy, supplements, integrative therapies, additional diagnostic considerations, and conventional treatment options. These are provided as consultative considerations for discussion with my pet’s regular veterinarian, who remains responsible for examination, diagnosis, prescribing, treatment, monitoring, and implementation of medical care.
• Recommendations are based upon the history, laboratory findings, medical information, and observations that I provide during the consultation. Because my pet will not be physically examined, there may be important clinical findings that cannot be identified through a telephone consultation.
• I am responsible for providing complete and accurate information about my pet’s medical history, current medications, supplements, diet, symptoms, and relevant diagnostic results.
• Swift River Veterinary PLLC does not provide emergency or urgent veterinary care. If my pet is experiencing an emergency, significant deterioration, difficulty breathing, collapse, uncontrolled bleeding, seizures, inability to urinate, severe pain, or another urgent problem, I will contact my regular veterinarian or an emergency veterinary facility rather than wait for a consultation.
• I understand that significant changes in my pet’s medical treatment should be discussed with and implemented through the veterinarian who has physically examined my pet and is responsible for my pet’s ongoing care.
